BOTH PROVIDERS AND PARTICIPANTS MUST COMPLETE APPLICATION FORMS IN ORDER FOR ELIGIBILITY TO BE DETERMINED. AFTER APPROVAL, CCAOA PAYS THE APPLICABLE FEE ASSISTANCE BASED ON ATTENDANCE SHEETS SIGNED BY BOTH THE PARENT AND PROVIDER. GRANTS TO SUBRECIPIENT ORGANIZATIONS ARE MONITORED WITH REGULAR CHECK INS AND REPORTING. PAYMENT SCHEDULES ARE TIED TO DELIVERABLES.
