SUMMARY OF DIRECT CHARITABLE ACTIVITIES FORM 990PF, PART VIII-A THE COLORADO HEALTH FOUNDATION'S ("THE FOUNDATION") DIRECT CHARITABLE ACTIVITIES SUPPORT WORK TO IMPROVE THE HEALTH OF ALL COLORADANS AND INCLUDED THE FOLLOWING IN 2021:PROVIDED TECHNICAL ASSISTANCE TO CLINICS THROUGHOUT THE STATE IN BUILDING AND IMPROVING THEIR TEAM-BASED CARE AND FINANCIAL PRACTICES, IN PARTNERSHIP WITH THE NONPROFIT FINANCE FUND AND JOHN SNOW, INC. LATE IN 2021, A NEW TECHNICAL ASSISTANCE PROVIDER JOINED THE TEAM, EPIC, FOCUSING ON SUPPORTING CLINICS IN DEVELOPING CULTURALLY RESPONSIVE CLINICAL AND PATIENT PRACTICES. THE ALSO COMPLETED ITS WORK WITH CHANGE MATRIX, WHICH HELPED THE FOUNDATION UNDERSTAND OUR IMPACT VIA INVESTMENTS IN "GROW YOUR OWN" WORKFORCE EFFORTS ACROSS THE STATE. THESE LEARNINGS WILL BE APPLIED TO THE FOUNDATION'S FUTURE STRATEGY IN CREATING AN EQUITABLE WORKFORCE THAT INCREASES ACCESS TO QUALITY PRIMARY CARE.CONTINUED WORK FROM 2020 TO SUPPORT CLIMB HIGHER (NOW KEYSTONE CENTER) IN BRINGING TOGETHER INTERMEDIARY ORGANIZATIONS THAT SUPPORT SCHOOLS, PARENTS, YOUTH AND SCHOOL STAFF TO DESIGN A MORE EQUITABLE APPROACH TO SOCIAL-EMOTIONAL LEARNING IN SCHOOLS (YOUTH AND YOUNG ADULT RESILIENCY PRIORITY). IN ADDITION, THE FOUNDATION CONTINUED SUPPORT TO THE JED FOUNDATION TO PROVIDE TECHNICAL ASSISTANCE TO COMMUNITY COLLEGES IN ENVISIONING AND IMPLEMENTING MENTAL HEALTH SUPPORTS FOR STUDENTS IN THEIR COMMUNITIES. THE FOUNDATION ALSO SUPPORTED NONPROFIT LEADERS WORKING ON YOUTH RESILIENCY TO SHARE BEST PRACTICES AND CREATE A NETWORK TO SUPPORT THESE LEADERS' HEALTH AND WELL-BEING. THIS GROUP ALSO PROVIDED DIRECT ASSISTANCE TO THEIR PEERS IN DEVELOPING WELL-BEING SUPPORT FOR THEIR STAFF. AT THE END OF 2021, THE FOUNDATION CONTRACTED WITH THE IMPROVE GROUP TO SUPPORT THE EVALUATION OF OUR MULTI-YEAR FUNDING OPPORTUNITY THAT FOCUSES ON SUPPORTING HEALTHY MINDS AND PROGRAMMING IN SUPPORT OF YOUNG PEOPLE'S SOCIAL-EMOTIONAL WELL-BEING.PROVIDED SUPPORT TO ASSESS AND BUILD COLORADO'S FAMILY, FRIEND AND NEIGHBORS (FFN) NETWORK (EARLY CHILDHOOD PRIORITY). SINCE FFN IS INFORMAL CARE, IT CAN BE CHALLENGING TO IDENTIFY PROVIDERS AND THOROUGHLY EVALUATE THEIR NEEDS STATEWIDE.
