POUDRE VALLEY HEALTH CARE PART V, SECTION B, LINE 6B: IN 2019, THE CHNA PROCESS FOR MEDICAL CENTER OF THE ROCKIES (MCR) AND POUDRE VALLEY HOSPITAL (PVH) WAS PREPARED IN CONJUNCTION WITH LARIMER AND WELD COUNTY HEALTH DEPARTMENTS. COMMUNITY-WIDE SUMMITS WERE FACILITATED BY COUNTY HEALTH DEPARTMENT STAFF, WITH MCR AND PVH PROVIDING SUBSTANTIAL INPUT INTO THE PLANNING AND EXECUTION OF THE SUMMITS. THESE SUMMITS INCLUDED APPROXIMATELY 150 REPRESENTATIVES OF COMMUNITY ORGANIZATIONS IN EACH COUNTY WHO ACTIVELY PROVIDED INPUT.FOLLOWING THE COMMUNITY SUMMIT MEETINGS LEADERSHIP TEAMS WERE ESTABLISHED BY THE COUNTY HEALTH DEPARTMENTS, PVH AND MCR TO CONTINUE THE CHNA PROCESS. THIS SMALLER LEADERSHIP TEAM INCLUDED REPRESENTATION BY: CITY OF FORT COLLINS, CITY OF GREELEY, CITY OF LOVELAND, COLORADO SCHOOL OF PUBLIC HEALTH, EARLY CHILDHOOD COUNCIL OF LARIMER COUNTY, HEALTH DISTRICT OF NORTHERN LARIMER COUNTY, LARIMER COUNTY DEPARTMENT OF HEALTH AND ENVIRONMENT, NORTH COLORADO HEALTH ALLIANCE, POUDRE SCHOOL DISTRICT, THOMPSON SCHOOL DISTRICT, THE FAMILY CENTER/LA FAMILIA, LARIMER COUNTY OFFICE ON AGING, NORTH RANGE BEHAVIORAL HEALTH, SUMMITSTONE HEALTH PARTNERS, SUNRISE LOVELAND COMMUNITY HEALTH CENTER, UNITED WAY OF LARIMER AND WELD COUNTIES, WELD COUNTY AREA AGENCY ON AGING, WELD COUNTY DEPARTMENT OF PUBLIC HEALTH, WELD COUNTY SCHOOL DISTRICTS.

NURSES ASSESS THE NEWBORN, PROVIDE THE MOTHER WITH LACTATION COUNSELING, SELF-CARE GUIDELINES AND ACHIEVE AN INCREASE IN THE MOTHER'S CONFIDENCE TO CARE FOR HER NEWBORN. ADDITIONALLY, COMPREHENSIVE CARE COORDINATION SERVICES ASSIST FAMILIES OF CHILDREN WITH SPECIAL HEALTH CARE NEEDS AS WELL AS VULNERABLE ADULTS WITH MEDICALLY COMPLEX, CHRONIC CONDITIONS. TO IMPROVE ACCESS TO MENTAL HEALTH SERVICES, PVH/MCR IS EXPANDING THE USE OF TECHNOLOGY TO PROVIDE TELE-PSYCH SERVICES TO RESIDENTS WHO ARE NOT ABLE TO ACCESS A LOCAL MENTAL HEALTH PROVIDER OR TO BRIDGE GAPS IN PROVIDER AVAILABILITY. FOR PERSONS WITH SUBSTANCE USE DISORDERS, PVH/MCR DIRECTS RESOURCES TO PROVIDING MEDICALLY ASSISTED ADDICTION TREATMENT AS WELL AS PARTICIPATING IN THE COLORADO ALTERNATIVES TO OPIOIDS TREATMENT PROTOCOLS. INCREASED SCREENING AND REFERRAL TO TREATMENT FOR PERSONS USING ALCOHOL HAS ALSO BEEN IMPLEMENTED.
