PART I, LINE 1A
COMPANION TRAVEL IS PROVIDED FOR THE BOARD OF DIRECTORS IN THE BOARD POLICY. THE VALUE OF THESE PAYMENTS ARE INCLUDED IN THE INDIVIDUAL'S INCOME AND APPROPRIATELY TAXED. TAX IDEMNIFICATION IS PROVIDED TO ALL EMPLOYEES FOR SERVICE AWARDS AND OTHER SMALL GIFT CARDS.
PART I, LINE 4B
THE CEO/EXECUTIVE VICE PRESIDENT IS ELIGIBLE FOR A SECTION 457(F) NON-QUALIFIED DEFERRED COMPENSATION PLAN. THE PLAN WAS ESTABLISHED IN 2008. TO DATE, NO AMOUNTS HAVE BEEN ACCRUED UNDER THE PLAN.
PART I, LINE 7
ALL EMPLOYEES ARE ELIGIBLE FOR A DISCRETIONARY INCENTIVE PAYMENT BASED IN PART ON THE FINANCIAL RESULTS OF THE ORGANIZATION. A DISCRETIONARY INCENTIVE PAYMENT WAS ACCRUED DURING THE FISCAL YEAR TO BE PAID OUT AFTER THE END OF THE FISCAL YEAR.
